The Provider Toolbox provides new and commonly requested information to DHS' provider community. DHS will continue expanding the section as part of agency efforts to enhance efficiency and communications throughout the services system.

DHS advises all of its providers that they must comply with the direction provided by the NYS Department of Health and suspend all referrals to adult homes identified on the DOH Referral Suspension List. The web link to this list can be found below.
