This form must be completed by a Licensed Master Plumber, Licensed Oil Burner Installer or Licensed Fire Suppression contractor for approval of all minor plumbing / oil burner installations, sprinkler / standpipe work as permitted and/or repairs requiring permits in accordance with section 28-101.5 of the New NYC Construction Codes. Used by licensed master plumbers and licensed fire suppression contractors to indicate the date and time of inspection/tests.

OP128: Ordinary Plumbing Work Report
This form is to be used by Licensed Master Plumbers (LMP) to submit up to 20 jobs, on-going or completed the previous calendar month, in accordance with §28-105.4.4 Ordinary Plumbing Work. This completed report form must be signed and sealed by the LMP testifying all work is code compliant. OP129: Report on Testing of Secondary Backflow Prevention Device
This form is to be used to report the test that is required on all Internal (secondary) Backflow devices. Secondary devices are RPZ’s or Backflow’s used for cooling towers, boilers, medical equipment, etc. These secondary backflow devices are recorded by DOB not DEP. The LMP completes section one (1) and three (3) and the certified tester completes section two (2). No P.E. or R.A. is necessary on this form.

Certification of Compliance with Color Coding
This form is to be used by Licensed Master Plumbers (LMP), Licensed Fire Suppression Contractors (LFSC), Registered Architects (RA), Registered Professional Engineers (PE), and holders of the Fire Department Certificate of Fitness to certify that all exposed standpipe and sprinkler components have been painted in accordance with Local Law 58 of 2009. This form must be kept on the building premises for inspection when requested by the Buildings Department and the Fire Department.
