WFU1 - Work Notice / Permit Application
This form shall be used when filing for approval of all proposed work requiring a permit in accordance with the NYC Building Code. It shall be accompanied by comprehensive plans describing the proposed work in detail, as well as, any information required by Small Business Services.

WFU4 - Place of Assembly
This form shall be used when filing for a Place of Assembly Permit whether it is new or changes to an existing one, in accordance with NYC Building Code.  It must be accompanied by plans describing the place of assembly in detail, as well as, any information required by Small Business Services.

WFU5 - Certificate of Completed Inspections
This form is to be submitted when requesting a Notice of Completion to sign-off an alteration that does not require a new certificate of completion. This is the equivalent to DOB’s letter of completion.

WFU6 - Equipment Use Approval
This form is to be submitted when requesting an Equipment Use Permit as per section 27-185 of the Building Code.

WFU8 - Affidavit for Certificate of Completion
This form is to be used when requesting a certificate of completion to sign-off an alteration or new building that requires a new certificate of completion. This is the equivalent to DOB’s certificate of occupancy.
