What's New for Providers


2009 Recommended Childhood and Adolescent Immunization Schedules

The Advisory Committee on Immunization Practices (ACIP) now recommends that all children 6 months through 18 years of age receive annual influenza vaccination. Children 6 months to <9 years of age should receive two doses of influenza vaccine the first season that they are vaccinated. Those who received only 1 dose of flu vaccine in their first flu season should get 2 doses in their second year of flu vaccination. Paper Reporting Forms No Longer Accepted

Effective January 1, 2009, all NYC providers are required to report immunizations administered to patients younger than 19 years to the CIR either by using the Online Registry or by sending electronic files extracted from billing, encounter or clinical information systems. New! Child & Adolescent Health Examination Form (CH205) is available via the Online Registry

New York City health examination forms required before attending a NYC school, day care program, after school program, and early intervention program and camp are streamlined into one new form, the Child & Adolescent Health Examination Form (CH205).  This new form replaces the following forms:  the School New Admission Examination Form (211S), the Bureau of Day Care Children’s New Admission Medical Record (318KA-1), the Bureau of Day Care Children’s Periodic/Follow-up Exam Medical Record (318KA), and the Health Record for Children in Day Camps & After School & Youth Centers (DRC7).

The Online Registry has made available the CH205 form, where the CH205 can be filled out online, then printed.  This official form is pre-completed with the patient’s name, date of birth, valid immunizations reported to the CIR, lead test dates and results, and the information entered by the provider as required by the health form and school program.

Starting January 1, 2009, providers can use only the CH205 form.
