HHC Connectx informs community providers about the wide array of clinical specialties and diagnostic procedures available at our hospitals, enables access to these services through our on-line referral service application and facilitates the participation of community providers in educational and CME opportunities available at our facilities. By developing operational excellence in referral and consultation management for community providers and their patients, this program intends to accomplish the following program goals:

• Promote access to services and continuity of care for patients receiving their care from both community and hospital-based providers
• Serve as a resource to community providers, offering value-added services that enhance the partnership between community providers and HHC networks
• Encourage effective communication between community and hospital-based providers
