Management Benefits Fund
Survivor Benefits


The Fund provides basic City health insurance coverage (including the optional rider), Dental, Superimposed Major Medical, and Vision Care benefits to the surviving spouse or domestic partner and eligible dependent children of deceased Fund members.

Please note that if the member was an employee of the State of New York, or the spouse/domestic partner is a City employee or retiree, survivors are then eligible only for MBF benefits (i.e., Dental benefits, Superimposed Major Medical coverage, and Vision Care benefits).

New York State or City Employees or Retirees
Survivors of New York State employees must contact the New York State Health Insurance Program for health benefits eligibility as a surviving spouse and may call the Department of Civil Service at (800) 833-4344 for further information.

Survivors who are also City employees must reinstate City health benefits for both the employee and eligible dependents by contacting their Agency benefits representative. Survivors who are also City retirees must reinstate City health benefits for both the retiree and eligible dependents by contacting the Employee Health Benefits Program Retiree Division at (212) 513-0470.

This benefit is provided for a period of three years beyond the date of a member's death. The cost of survivor coverage is fully paid for by the Fund.
