LICENSES | How to Become an Industry Representative

An industry representative is a person who is granted permission by the TLC to represent a respondent before the TLC's tribunal. Candidates must complete the Application for Industry Representative, and mail it or deliver it to the below address:

Adjudications Division
32-02 Queens Boulevard, 3rd Floor
Long Island City, New York 11101.

All candidates must take and pass a written examination. The examination will test your knowledge of the Commission rules and procedures, the standards of conduct for Industry Representatives, and other codes and regulations applicable at the Commission's tribunals.

If you are successful in achieving a passing grade of at least seventy percent (70%) you will then be required to be fingerprinted. This will be done at our Long Island Facility located at 32-02 Queens Boulevard, on the 2nd floor. At that time you will need a U.S Postal Money made payable to N.Y.S.D.C.J.S. in the amount of ($75.00) seventy five dollars.
