What is Case Management?
Case management is a journey alongside participants, where you are there to provide guidance and support. Taking this journey together requires trust, communication, clear goals, and the ability to collaboratively problem-solve along the way.

The Purpose of the Toolkit
The toolkit is designed to provide supervisors and staff at DYCD-funded community-based organizations with a framework for understanding the case management process and implementing case management best practices in their respective programs. The toolkit serves as a basic guide to help case managers and their supervisors identify key principles of effective case management and techniques to build successful relationships with program participants to support them in achieving their goals. Throughout this toolkit, learners will be guided by DYCD program standards and guidelines as well as recommended practices to support successful outcomes for programs and participants alike.
About the Case Management Toolkit
Vibrant Emotional Health collaborated on this toolkit in partnership with the New York City Department of Youth and Community Development. Vibrant Emotional Health (formerly the Mental Health Association of New York City) is a nonprofit organization that addresses mental health needs in New York City and across the nation. Vibrant promotes mental and emotional wellness for all by providing direct service programs for youth and adults, advocating for policies that improve outcomes for those with mental and behavioral disorders, and developing innovative training and education for practitioners, youth workers, and families.
