Staff providing case management servicesDefinitionTypes of hands-on assistance offered by a program (ex: job training, academic enrichment, legal services, mental health services, food stamp application guidance, college application support) can play a very important role in building and strengthening each person’s unique circle of support. By regularly communicating with all providers working to support the participant, this network of support grows stronger and allows for all perspectives, including the participant’s, to be shared in order to determine solutions for moving forward. When services are provided in coordination with other services the participant is involved with, the participant doesn’t have to provide the same information over and over again.
