- What are the participant’s goals, both long term and short term?
- What are their aspirations and dreams (Long-term GoalsDefinitionAn objective that an individual wants to accomplish in the future that requires planning and time, perhaps several years, and likely includes a succession of several action steps)?
- What are their goals for this program term?
- What strengths can help them achieve their goals?
- What resources and supports needs to be accessed in order to reach the goals?
- What does the PARTICIPANT want or need to achieve first?
- What steps can be taken immediately toward the goal?
