Individualized Service Planning with the participant is the process of working together with case management staff to figure out HOW the goal will be accomplished. It includes outlining action steps that will be taken and sets milestones that will demonstrate progress. Each step needs to have a timeframe and specify who is responsible for completing this step. We want to help those with whom we are working to get a clear picture of what the end goal will look like and how they will know when they have achieved this goal.
The written Individualized Service Plan must include the following elements for each goal:
- What are the steps to take to reach this goal?
- By when will each step be accomplished?
- Who will be responsible for each step?
- What will be the evidence/proof that the goal has been achieved?
