The toolkit is organized in order of how a “typical” case management relationship would unfold over time, starting with the first time you meet a participant (Starting the Journey) to your final meeting (Concluding the Journey).
We understand that sometimes things may happen out of order. For example, you may determine during your initial assessment that a referral to a certain organization may be helpful to your participant, even though referrals are not discussed until later in the toolkit. That is why it is important to explore the toolkit in its entirety first so you are familiar with everything it has to offer.
