Reassessment offers the opportunity to evaluate the impact of your current plan and activities to date, assess participant progress, and identify barriers to the full attainment of the participant’s goals. In addition to triggering an update of the current plan, reassessment allows staff to determine whether current services or service levels are appropriate or if the participant should be offered alternative services.
A reassessment is performed 3 months after the initial assessment, or earlier if barriers to implementation are identified or are required by program guidelines (e.g., in programs for runaway and homeless youth, reassessments must take place every 30 days). The reassessment period allows for a re-evaluation of participant strengths and needs, identifies changes since the initial or most recent assessment, and affirms ongoing or new goals, objectives, and activities.
The written plan should be revised following a reassessment but it may also be updated between reassessments to reflect changes in goals, case management activities, and participant circumstances.
