Ensure all service providers come prepared with information from their agencies\/teams.<\/li>\n\n\n\n<li>Review one participant at a time.&nbsp;<\/li>\n\n\n\n<li>Clarify roles for the meeting: facilitator, timekeeper, and notetaker.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p><strong>Phase #2 \u2013 Information Gathering<\/strong><\/p>\n\n\n\n<ul>\n<li>Each person around the table shares critical details of barriers, <span id=\"su_tooltip_64f7ed8c9ce13_button\" class=\"su-tooltip-button su-tooltip-button-outline-yes glossaryWord\" aria-describedby=\"su_tooltip_64f7ed8c9ce13\" data-settings='{\"position\":\"top\",\"behavior\":\"hover\",\"hideDelay\":0}' tabindex=\"0\">Strengths<\/span><span style=\"display:none;z-index:100\" id=\"su_tooltip_64f7ed8c9ce13\" class=\"su-tooltip glossaryWord\" role=\"tooltip\"><span class=\"su-tooltip-inner su-tooltip-shadow-no\" style=\"z-index:100;background:#222222;color:#FFFFFF;font-size:16px;border-radius:5px;text-align:left;max-width:300px;line-height:1.25\"><span class=\"su-tooltip-title\">Definition<\/span><span class=\"su-tooltip-content su-u-trim\">Positive qualities that participants have as a result of their unique personal, professional, and general life experiences (ex: a positive attitude, persistence, agility, work ethic)<\/span><\/span><span id=\"su_tooltip_64f7ed8c9ce13_arrow\" class=\"su-tooltip-arrow\" style=\"z-index:100;background:#222222\" data-popper-arrow><\/span><\/span>, and solutions. <em>Do not get into storytelling or sharing unnecessary details.<\/em><\/li>\n\n\n\n<li>Ensure that the timekeeper is keeping to the time schedule. Some participants may require more time than others.<\/li>\n\n\n\n<li>Facilitator summarizes and confirms the strengths, barriers, and service needs with the group.<\/li>\n<\/ul>\n\n\n\n<p><strong>Phase #3 \u2013 Problem-Solving and Action Planning<\/strong><\/p>\n\n\n\n<ul>\n<li>Identify available resources and develop an action plan for each participant discussed.<\/li>\n\n\n\n<li>Plan next steps and ensure roles and responsibilities are clear.<\/li>\n\n\n\n<li>Send out action items immediately following the meeting, so everyone knows who is responsible for each item.
