Action Step:<\/strong> a clear task an individual can reasonably accomplish on their own (ex: identify three job opportunities, complete a job application, attend tutoring sessions)<\/p>\n\n\n\n

Case Closure:<\/strong> the process by which a service recipient and service program end their formal relationship <\/p>\n\n\n\n

Cultural Humility<\/strong>: a process of openness, self-awareness, and self-reflection when interacting with diverse individuals, resulting in mutual empowerment, respect, partnerships, optimal care, and lifelong learning (from the National Child Welfare Workforce Institute)<\/p>\n\n\n\n

Long-term Goal:<\/strong> an objective that an individual wants to accomplish in the future that requires planning and time, perhaps several years, and likely includes a succession of several action steps<\/p>\n\n\n\n

Motivational Interviewing: <\/strong>a participant-centered counseling style to help participants with behavior change by helping them explore and resolve ambivalence (i.e., uncertainty about their goals or factors holding them back from achieving them)<\/p>\n\n\n\n

Needs:<\/strong> resources or services an individual requires to utilize their strengths in reaching their goals (ex: childcare, resume support, internship stipends)<\/p>\n\n\n\n

Positive Youth Development:<\/strong> an intentional, prosocial approach that engages youth within their communities, schools, organizations, peer groups, and families in a manner that is productive and constructive; recognizes, utilizes, and enhances young people\u2019s strengths; and promotes positive outcomes for young people by providing opportunities, fostering positive relationships, and furnishing the support needed to build on their leadership strengths (from youth.gov)<\/p>\n\n\n\n

Priorities:<\/strong> immediate action steps that are necessary in the short term to reach short- or long-term goals<\/p>\n\n\n\n

Program Outcomes:<\/strong> the actual results of planned efforts that a program undertakes<\/p>\n\n\n\n

Resources:<\/strong> tools, materials, or other assets that can support a participant\u2019s ability to reach their goals (ex: access to an online learning module, childcare, MetroCard)<\/p>\n\n\n\n

Services:<\/strong> types of hands-on assistance offered by a program (ex: job training, academic enrichment, legal services, mental health services, food stamp application guidance, college application support)<\/p>\n\n\n\n

Short-term Goal:<\/strong> a specific, measurable, achievable, relevant, and time-framed goal that can be accomplished in the near future <\/p>\n\n\n\n

Strengths:<\/strong> positive qualities that participants have as a result of their unique personal, professional, and general life experiences (ex: a positive attitude, persistence, agility, work ethic)<\/p>\n\n\n\n

Support Systems<\/strong>: networks of individuals or programs that can aid a participant in reaching their goals by providing resources or services (ex: family, friends, religious community, the New York Public Library, Human Resources Administration)<\/p>\n\n\n\n

Transition Plan:<\/strong> a set of steps intended to help the program participant maintain support and progress when they are no longer regularly participating in the program<\/p>\n\n\n\n

Trauma-informed Practices<\/strong>: practices that acknowledge the prevalence and variety of trauma that exists in society today and promote a culture of safety, empowerment, and healing<\/p>\n","protected":false},"excerpt":{"rendered":"
