Change of Business Address

  • LIC7 Application (Original, Typed, & Notarized)
  • Check the Change/Reissue box in Section 1
  • Original lease, deed, bank statement, or utility bill in the business name showing the new company address
  • Insurances (Workers Comp, Disability, & General Liability)
    *Note: Anyone listed on the application or a Filing Representative can submit the documentation.*

Change of Business Name

  • LIC7 Application (Original, Typed, & Notarized)
  • Check the Change/Reissue box in Section 1
  • Certificate of Amendment Certified by New York State Department of State
  • Original lease, deed, bank statement, or utility bill in the new company name
  • Original Safety Registration Card
  • Insurances (Workers Comp, Disability, & General Liability)
  • $50.00 Re-Issue Fee

Change of Business Telephone Number

  • LIC7 Application (Original, Typed, & Notarized)
  • Check the Change/Reissue box in Section 1
  • Notarized letter on company letterhead that indicates the old business telephone number and the new number
    *Note: Anyone listed on the application or a filing representative can submit the documentation.*

Re-Issue Lost Registration Card

  • LIC7 Application (Original, Typed, & Notarized)
  • Check the Change/Reissue box in Section 1
  • Police report for the lost or stolen registration card
    *Note: Primary principal must appear in-person to submit the required documentation.*
