All violating condition(s) must be corrected and certified as corrected with the NYC Department of Buildings. To certify correction, you need an NYC.ID account to login to DOB NOW. Log in at nyc.gov/dobnow and select the BIS Portal.
From the DOB NOW BIS portal, select +Certificate of Correction Review Request and enter the required fields including the Summons number. Select Save and then in the Documents section, upload completed applicable AEU2, AEU3321 or AEU20 forms and any other required documents. After completing the Statements & Signature section, select Submit. The request will not be reviewed by AEU until the Submit button is selected and confirmation is given for it to be submitted.
An email notification will be sent to the applicant with the status of the review. If the request is disapproved, a new request must be submitted.

The paper Certificate of Correction affidavit and statement forms (AEU2, AEU3321 and AEU20) can no longer be sent or delivered in-person to the Administrative Enforcement Unit. To submit a Certificate of Correction request, you need an NYC.ID account to log in to DOB NOW. Log in at at nyc.gov/dobnow and select the BIS Options portal. DOB accepts a Certificate of Correction any time after the summons is issued, except summonses issued for filing a Certificate of Correction containing materially false statement(s). In such a case, you must attend your hearing before you can certify correction with DOB.

AEU20: Statement in Support of Certificate of Correction
Use this form when preparing the required statement(s) to accompany the Certificate of Correction (AEU2 Form).
AEU3321: Certificate of Correction & Statement in Support of Site Safety Training
The AEU3321 form must be used to certify the correction of summonses issued for failing to comply with training requirements pursuant to BC Section 3321.1 and/or Section 3321.2. The form combines the AEU2 (Certificate of Correction) and the AEU20 (Statement in Support). The AEU2 and AEU20 forms will not be accepted for summonses related to the site safety training requirements. Please be sure to use the AEU3321 form with the Rev. 4/20 date.
