Fill out this form to notify the NYC Department of Consumer and Worker Protection (DCWP) when you file a case in court or a private arbitration for violations of the Paid Safe and Sick Leave Law (Earned Safe and Sick Time Act), Fair Workweek Law, Temporary Schedule Change Law, Delivery Worker Laws, Freelance Isn't Free Act, or another law that DCWP enforces. If the law requires you to serve on DCWP a copy of the complaint you filed in court, you can complete this online form to satisfy that requirement.
