Death Reporting

For Funeral Homes and Families of Decedents

Funeral homes work with informants (family members or other persons who are very familiar with the decedent) to collect data about the deceased for reporting on the death certificate. Funeral Directors can download and distribute the tools below to help informants better understand the information required for reporting.
