• A completed Certificate of Fitness Application Form (PDF)
• A letter of employment.

• If required, a copy of your diploma or academic transcript.
  (Degree issued from outside the USA or is not in English shall be evaluated by an independent evaluation service accepted by the FDNY)

• Two forms of identification, at least one must be government-issued photo identification, such as a driver's license or a passport.
