You have the right to request a reasonable accommodation if you feel that a program or service is not accessible to you. A request for a reasonable accommodation should be in writing and contain information including the name, address, and phone number of the applicant and the date and description of the problem. Alternative means of filing requests will be made available for persons with disabilities upon request. The request should be submitted by the applicant or the applicant’s designee as soon as possible to the Disability Services Facilitator or ADA Coordinator, at nyc.gov/contactdofeeo or by calling 311 or 212-639-9675 (TTY).
The ADA Coordinator or his designee will contact the applicant to discuss the request and the possible resolutions. Within 10 business days of the receipt of a completed request for a reasonable accommodation, the ADA Coordinator or his designee will respond in writing, and where appropriate, in a format accessible to the applicant. The response will explain the position of the Department of Finance and offer options for resolution of the complaint.
