• Supply and Service Contractors

Federally assisted contracts:

• Prime or subcontracts

City or State funded contracts:

• Prime contracts over $1M
• Subcontracts over $750k
• Subcontracts under $750k

Supply and Service Contractors

The following forms must be submitted for construction contracts over $100k. See instructions.

Federally assisted contracts:

• Prime or subcontractors with 50+ employees
