Vehicle Plate Notification Form
Beginning Thursday June 7th, 2018 the Plate Notification form will only be available through the TLC Upload Portal and can be submitted by the vehicle owner or affiliated Base. A plate notification must be submitted to the TLC before a TLC vehicle inspection can be scheduled.

Note: If you have submitted your vehicle application to the TLC, you are affiliated to a Street Hail Livery (SHL) Permit, and you are ready to Hack Up your vehicle, go to your base to notify the TLC that you have received your license plates. You will receive a Hack Up letter in the mail. Bring your vehicle with the Hack Up letter to our Inspection Facility at Woodside and the inspection will be completed there.
