Harlem Health Advocacy Partners

Many people living in East and Central Harlem, especially residents of public housing, are sicker than other New Yorkers. This is because housing, access to healthy foods, safe parks and other neighborhood conditions affect people's health.

To improve well-being in these neighborhoods, the Center for Health Equity and Community Wellness has contracted with Public Health Solutions to implement Harlem Health Advocacy Partners with health insurance navigation support through Single Stop's Health Advocates.

Services

Community Health Workers can help you:

  • Set health goals
  • Manage existing chronic diseases
  • Connect to local health and social services

Health Advocates can help you:

  • Find affordable or low-cost health insurance
  • Understand medical bills
  • Review health plan options

Group Wellness Activities

Harlem Health Advocacy Partners also organizes in-person and virtual group wellness activities, such as walking groups, peer support groups and workshops to help you quit smoking. These activities are open to all East and Central Harlem residents.
