All organizations receiving a Fiscal 2024 CDF grant must report on their funded services in order to retain the funds paid, receive any payments due, and remain in good standing in the future.
Final Reports are reviewed in order of submission on a rolling basis; organizations are encouraged to submit well in advance of the final deadline. Final Reports are subject to review by the Agency and additional information may be requested prior to approval.
All grantees must complete an online CDF Final Report form that details programming supported in Fiscal 2024 (organizations should log into the online portal by clicking the link at the bottom of this page). The online Supplemental & Scope Change form have been merged into a single submission form within the final report. These materials will only be accepted via the online form. The form is due on or before Thursday, August 1, 2024 at 5:00pm.
