If the notification is for an application for a new on-premises license, or an existing licensee that intends to alter or upgrade its license, Community Board 9 requires separate forms that must be filled out and submitted to the board to be reviewed by the Board's Public Safety Committee. To obtain the Board's SLA Questionnaire and Stipulations forms, please contact the Board Office 718-778-9279 or bk09-1@cb.nyc.gov.
Once the completed form has been returned to the Board's Office, the establishment will be placed on the agenda for the next meeting Board's Public Safety Committee. The committee usually meets at 7:00pm on the second Thursday of the month. To confirm the meeting details, please contact the office at 718-778-9279 or bk09-1@cb.nyc.gov. To be added to the Public Safety Committee's agenda, your documentation must be submitted to the Board Office no later than 12PM the Friday prior to the committee meeting.
