Other Rent Freeze forms

In addition to the initial and renewal Rent Freeze Program applications, you may at times need to provide additional information. Below you will find some of the most commonly used Rent Freeze Program applications and forms.

Use this form if you have moved into another rent-regulated unit and would like to transfer your SCRIE benefit.

Use this form to appeal a Rent Freeze Program decision.

Use this form to apply to take over a Rent Freeze benefit from a beneficiary who has passed away or permanently moved.

Use this form to transfer from DRIE to SCRIE, or from SCRIE to DRIE.

Use this form in place of a lease if you have been unable to obtain a signed lease from your landlord.

Use this form to report a change in your household or select a new tenant representative.

For new Rent Freeze Program applicants who are income tax filers, your total household income is the federal adjusted gross income (AGI) from all of the household’s tax returns, minus taxable IRA distributions and withdrawals. You also have the option to use the worksheet below. If you do not file taxes, you can complete this optional worksheet to calculate your income.

Use this form to apply for a redetermination of your frozen rent amount if you had a permanent loss of 20% or more of your household income since your last approved application.

To request more time or other assistance, you may call 311, write to DOF, schedule a virtual appointment, or complete a Request for an Extension of Time Due to a Disability or Physical or Mental Impairment. You may need to submit medical documentation supporting your request.
